April 6, 2026 Phone System Improvements

Based on feedback from our patients, we have updated our phone system to improve access and better connect you with your clinic team. Patients can now connect directly with their clinic reception by selecting the appropriate option from the automated menu.

When calling, please listen carefully to the prompts and select your clinic from the menu options to ensure your call is directed to the correct team as quickly as possible.

April 6, 2026 Community Social Work Support Now Available

We are also pleased to welcome Josh Lambert, Community Social Worker, who is available to assist patients with community‑based needs and questions. Josh can help connect patients with resources such as:

Housing supports

Food and clothing resources

Financial or social assistance programs

Other community services that may be available to you

Patients interested in accessing community social work support are encouraged to contact the clinic for more information.

April 6, 2026 New Partnership with the Ontario Disability Support Program (ODSP)

We are excited to announce a new partnership with the Ontario Disability Support Program (ODSP). Through this partnership, an ODSP worker is available on‑site at Queen’s Family Health Team, 220 bagot Street on a bi-weekly basis to assist patients with ODSP‑related needs. Support may include help with:

Completing patient self-report sections of forms

Answering common questions

Information about available ODSP medical and financial benefits.

There will be an on-site ODSP worker available every other Tuesday starting April 7, 2026 from 8:30am to 12:00pm. Appointmenst and drop-ins are welcome.

Dec. 2024 - Queen’s Family Health Team updated its phone system. New process for appointments and care inquiries.

In early December 2024, hospitals across southeastern Ontario transitioned to a new health information system to enhance regional care and coordination.
However, this important upgrade has temporarily delayed the delivery of some imaging reports. Specifically, reports from KHSC and the surrounding hospitals for tests on or after December 7th may take longer to reach us.
If you had a test at one of the hospitals during this time and have not yet received your results, please rest assured this delay is temporary. We are working closely with our hospital partners and their teams to resolve these delays quickly.
